LTS Liability and Refund Waiver 2023 - Text

GENERAL LIABILITY WAIVER As a participant at Willowbrook Ice Arena, I recognize and acknowledge that there are certain risks of physical injury. I agree to assume the full risk of any injuries, damages of loss, which I may sustain as a result of participating in any and all activities, connected with or associated with such programs. I agree to waive and relinquish all claims I may have against Willowbrook Ice Arena, and its officers, agents and members as a result of participating in any of the programs offered. I do hereby fully release and discharge participants and Willowbrook Ice Arena, and their officers, agents and members from any and all claims from injuries, which include death, damages or less which I may have or which may accrue to me on account of my participation in the program. I further agree to indemnify and hold harmless and defend Willowbrook Ice Arena and its officers, agents and members from any and all claims resulting from injuries, including death, damages and losses sustained by me and arising out of, connected with, or in any way associated with the activities of the program. By entering this facility, I am aware that I agree to fully accept all known and unknown risks, including the potential risk of exposure to respiratory illnesses such as the coronavirus (COVID-19). The coronavirus is primarily transmitted via exhaled respiratory droplets, most often through coughing and sneezing. These droplets can travel up to six feet and are more commonly transmitted between persons rather than from equipment to persons. I have read and fully understand the above Waiver and Release of all Claims.

REFUND POLICY I acknowledge all sales are final and NO REFUNDS WILL BE GIVEN AFTER THE PARTICIPANT ATTENDS TWO CLASSES. If a parcipant decides to drop from the session after completing ONE class, the participant will receive a refund for the session minus the prorated cost of one class and a $20 processing fee. No refunds will be given, for any reason, aer the third week of the session.
